Help us celebrate and recognise Community Pharmacy achievements and value across Devon for the previous year. The awards ceremony will take place on 11th November 2019.

Our five awards are;

  • Community Pharmacist of the Year
    • For the Community Pharmacist who has gone above and beyond to deliver patient care within their local community
    • Nomination Form
  • Community Pharmacy Team of the Year
    • For the Pharmacy team that has delivered an outstanding service to their local community
    • Nomination Form
  • Pharmacy Team Member of the Year
    • For a member of the Pharmacy Team who has made an outstanding contribution to local patient care or to the running of the pharmacy
    • Nomination Form
  • Multi Disciplinary Working
    • For a pharmacy or a member of the pharmacy team that has worked across boundaries to deliver innovative care
    • Nomination Form
  • Innovation of the Year
    • For a pharmacy or member of the pharmacy team that has delivered an innovative approach to the adoption of future working and the challenges ahead particularly with the introduction of Primary Care Networks
    • Nomination Form

Nominations will be open for the awards from July to the end of October. Nominations can come from any source, eg patient, CCG, Owner, Company etc. A nomination form can be posted to the LPC (Devon LPC, Deer Park Business Centre, Haldon Hill, Kennford, Exeter EX6 7XX)  or can be emailed to the LPC office.  Send to: kathrynj@devonlpc.org

Awards will be open to anyone who works in a Community Pharmacy within Devon.

Once all the nominations are in a panel will review them, and a short list of three will be created for each award and each team/colleague will be informed and invited to the AGM on 11th November at Exeter Court Hotel, where Simon Dukes, CEO of PSNC will once again be attending.

On the evening a short statement regarding each finalist in a category will be read out.  All three finalists will receive a certificate to celebrate their achievements and then a winner will be announced who will receive an Award and prize.

Photos will be taken and posted on social media and will also be available to go in the local press
