We have received nominations for all the categories, however, due to a change in date of the awards evening, we are able to extend the closing date to Monday 12th November 2018

Community pharmacies across Devon deliver excellent patient and customer care every day; yet often we fail to stop and celebrate the successes achieved by our teams. The LPC is delighted to announce our very first Community Pharmacy Awards Ceremony which will showcase and celebrate your successes.

The five award categories are:

  • Community Pharmacist of the year
    • For the Community Pharmacist that has gone above and beyond to deliver patient care within their local Community
  • Community Pharmacy team of the year
    • For the Pharmacy Team that has delivered and supported the local community
  • Pharmacy Team member of the year
    • For a member of the Pharmacy team that has made a difference in local patient care
  • Multi Disciplinary Working
    • For a member of the Pharmacy team that has worked cross functionally with their local Healthcare team to deliver innovative care
  • Pharmacy Service/Innovation of the year
    • For the team individual that has delivered exceptional Pharmacy Service or Innovation to their local Community

Nominations are sought during August to end of September. Nominations can come from any source, e.g.  patient, CCG, Owner, Company, another member of your team or self nomination.  Awards will be open to anyone who works in a Community Pharmacy within Devon.  Nomination forms for each category are listed below;

Closing date Monday 12th November 2018.   

The runners up and winners will be announced at the annual contractors meeting on Monday 26th November 2018, where you will also have to chance to meet Simon Dukes the new CEO of PSNC.
