Transfer of Care Around Medicines (TCAM) (formerly know as Medicines Support Service)

A refreshed TCAM service is coming to you from the Northern Devon Healthcare Trust. As you know, this process aims to improve the communication and care for our patients around medicines after they leave hospital.   With the move away from faxing information within the NHS electronic transfer of information is becoming all the more important.

It is important ALL community pharmacy teams book-in and attend this event as it represents an important milestone for us as a profession and will lead to better integration with local systems.

Learning outcomes from the session will include:

  • Key benefits to patients, hospital and community pharmacy and the newly emerging primary care networks
  • Best practice sharing from active TCAM and NMS providers and example case studies to discuss and learn from
  • Operational aspects
  • Pharmacy team engagement
  • Future development opportunities

Note: Cancellation of booking policy: If you don’t turn up to an event despite booking a place or giving 3 days’ notice of cancellation in writing, the LPC will send out an invoice for £50.00 for each booked place where no attendance is made.
