Medicines Support Service – Improving Transfer of Care in Plymouth

This is an exciting move towards improving patient safety and firmly establishing community pharmacy as a part of the patient’s pathway as well as being your opportunity to view the hospital pharmacy enhancing relationships and providing a valuable networking opportunity.

Derriford Hospital Pharmacy with partners Devon Local Pharmaceutical Committee and NEW Devon CCG are pleased to announce a launch event, training and networking session aimed at enhancing professional relations between Hospital and Community Pharmacy while launching PharmOutcomes to share patients’ discharge documentation.

This is your chance to:

  • Visit the hospital pharmacy.
  • Understand the Medicines Support Service and the role of PharmOutcomes
  • Understand how it will work
  • Understand what you need to do
  • View and discuss potential follow up actions
  • Network with new and old colleagues

We are expecting this to be a popular event so to avoid disappointment please register ASAP, and by October 6th.

Unfortunately attendance has to be restricted to a maximum of two attendees from each pharmacy.

Parking will be free in car parks C or D – on entry and exit to the car park say you are attending the pharmacy event into the intercom and the barrier will be lifted.

Cancellation of booking policy: If you don’t turn up to an event despite booking a place or giving 3 days notice of cancellation in writing, the LPC will send out an invoice for £50.00 for each place booked where no attendance is made.
