 Significant Event Reporting

Event reporting provides valuable learning to make the NHS safer for patients. Only a small percentage of events result in actual harm. No harm and low harm incidents provide an opportunity to learn and develop strategies to minimise the risk of preventable harms. Completing an event reporting form can help with the implementation of any learning and potential future errors can be reduced –
